This is an example plan. Use it as a starting point for your own Goal Map.
Choose one cohort outcome and three progress signals
Make the outcome and signals observable inside the program window.
Set privacy boundaries
Define what leaders, mentors, program staff, and members can see before data starts moving.
Capture each member's starting point
Use one consistent baseline before mentoring activity begins.
Give every member a first Goal Map and named mentor
Confirm each participant knows the first milestone, next action, and human support path.
Review stalled first milestones weekly
Look for support needs and stalled signals without opening private mentoring content.
Record mentor interventions and the next agreed action
Keep follow-through visible between meetings without exposing private conversation detail.
Collect outcome evidence at the final milestone
Use artifacts, confirmations, or measured results rather than self-reported completion alone.
Review where members stalled
Compare the interventions that helped with those that did not and look for program-level patterns.
Update the next cohort design
Turn the outcome evidence and learning review into specific program changes with owners.
